A setback can leave families unsure what to do next. Start by checking immediate safety, then consider what support is appropriate for the situation.

Reassess the next step with the right support.

A return to substance use does not erase previous progress. NIAAA explains that a return to drinking can signal that a treatment plan needs adjustment. The immediate response depends on the person’s safety and medical needs. After urgent needs are addressed, contact the treating professional to discuss what changed and what support may fit now.

Practical starting points

  • If someone may be in immediate danger or needs urgent medical care, contact emergency services.
  • When the situation is not an emergency, take time to understand what changed and what the person is willing to consider.
  • Avoid making major decisions in the middle of a heated argument when it is safe to pause.
  • A counselor or family support professional can help you think through possible next steps.

A conversation example

For a nonemergency conversation, a family might say: ‘We want to understand what happened and what help makes sense now. Is there someone on your care team you would like to contact?’ Avoid turning one conversation into a demand to solve the entire future. The family can also arrange its own support while decisions are being made.

Before your next conversation

  • Is there an immediate medical or safety concern?
  • Which treating professional or support person can help reassess the plan?
  • What practical needs and family boundaries should be discussed next?
